Could anyone please help me on this surrender tag?

 

On the picture on left, I am picking out "*something*da Kiyo" and not sure on the smaller print

 

On the right I can only pick out 'to' and 'ichi'

 

So I am essentially no help and completely lost. Posted above for both comic relief as well as showing that I am at least looking and trying before asking for help. Thanks!

One side says 1 Japanese sword, mumei...The other side has a name, Iwata Kiyoshi, and address, adachi ku senju san chome 63. That is in Tokyo...

Could someone tell me what the topmost character on the left in the first picture is? (A?) I saw the same one on a nakago recently and I couldn't track it down.

足 - usually reads soku/ashi.
