Can anyone translate a “surrender tag”?

This is a sword I’m looking at tomorrow. These are the only pictures I have so far. I’m pretty excited about it but looking to see if anyone smarter than me can translate the wooden tag. 
 

interested in anyone’s opinions and thoughts as well.

Hi Gunner, 

the surrender tag is a hard read because the photos have uploaded upside-down. Any chance you can rotate them and repost them? You might want to snag a photo of the tang and signature with any stamps too so that you know exactly what you’re looking at.

The tag is badly rubbed, hovering on the edge between illegible and barely legible. It seems to give an address and a name, but to get the exact reading would take someone hours of detective work!

I'll pick the low hanging fruit to start off with - I think I can see Shitahara in the middle line: 下原

Bottom of the left hand column, perhaps the owner's name: ?泉景原 - ?sen or ?izumi Kagehara (but not 100% sure).

下原免 = Shimobaru-men   (an address in Nagasaki)

Transportation tag, I've had a few usually just owners name and address.

1000 yen was not an insignificant amount back then, it may be a sword with potential.
