大和大掾藤原氏重 - Yamato Daijo Fujiwara Ujishige.

 

There were three generations of smith with this name and title and their work periods are as follows. From Sesko's Compendium:

 

Ujishige (氏重), 1st gen., Manji (万治, 1658-1661), Harima
Ujishige (氏重), 2nd gen., Genroku (元禄, 1688-1704), Harima
Ujishige (氏重), 3rd gen., Genbun (元文, 1736-1741), Harima
