Here is a Jingasa. I have no idea what family the mon represents.

Here are some variations of the Yotsu-me (Four-eye pattern), similar ones but not the one in question could be find in Hawley's Mon-book, called Me-yui (eyes) there.

Often jingasa will have been refurbished, or even built from new. Difficult to judge from on top. The underside usually gives the most information, like yours.

Kinda “Bajo Jingasa”. 
My gut feeling suggesting the mon was added later.....
Anyway, couldn’t find anything matching in my references. But a large mon book is on route to me. As soon it arrived, I’ll have a second look!
