Tsuba Translation Request

I have this tsuba and I have found a very similar tsuba in a book, could you help me to translate the comment and perhaps the signature

Take this with a pinch of salt but at first glance it looks like:

Kunimoto Naoshige 国本尚茂

 

The inscription says:

Kawari　okidaiーgata　変ã‚ã‚Šç½®å°åž‹unusual base/pedestal shape

 

It certainly is very unusual. Looks almost classical. I wonder what kind of koshirae it was originally on?
