Could You Translate A Date For Me?

Ive got the signature as "Fujiwara Sadayuki" (correct me if wrong) but I understand that's a common name. Could you translate the date so I can narrow it down?

The second isn't a date. It's another name X da/ta Kiyoyoshi

It could be who it was made for. I doubt the ta/da kanji is for rice field. I don't know what the first kanji is but it'll be the first part of a family name - Like Fujita or Yamada or something (but it's not those).
