Name (Family name) - 渡邊 (Watanabe/Watabe)

and

Address - 大西町 紺原X (Ohnishi-cho Konbara+something) - a town in Ehime prefecture

I guess that the tag belonged to one of previous owners.

Oh, now I see.

The "tag" is a metal plate screwed to the top or bottom of the shirasaya containing the Muramasa sword. The ink mei is "Nao ..?.. Saku" is probably the name of the sayashi and the metal tag is probably the name of the collector/museum/inventory the sword previously belonged to.

Hi Kunitaro-san. I also spent 2h looking at Kanzan Sayagaki and it did not look right, and had the same thoughts. If it had new papers it would sell for a bit more than the asking price :D

Yes, Jean san is right.

purchasing the sword with old papers (Kicho,Tokubetsukicho, Koshu-TK etc), especially through internet.

we can always confirm or ask seller to guarantee of issuing new Hozon paper.

for Mumei blade, the attribution could be change, or some are possible failure of issuing new Hozon.
